Frequently Asked Questions

Everything you need to know about Direct Primary Care and Ash & Elm Wellness.

What is Direct Primary Care?

Direct Primary Care (DPC) is a healthcare model where you pay a flat monthly membership fee directly to your provider — no insurance billing, no copays, no surprise charges. Your membership covers comprehensive primary care including unlimited visits, direct provider access, preventive care, chronic disease management, and in-office procedures. It’s medicine the way it should be: personal, transparent, and focused entirely on you.

Is this insurance?

No. DPC is a membership-based primary care model, not insurance. Your monthly fee covers primary care services at Ash & Elm. It does not cover hospitalizations, emergency room visits, specialist care, or imaging outside the clinic. Many members pair their DPC membership with a low-cost catastrophic or high-deductible insurance plan for major medical events.

What does membership cost?

Monthly membership rates are based on age: Ages 18–30: $75/month | Ages 31–45: $90/month | Ages 46–60: $100/month | Ages 61+: $110/month. Pediatric add-ons are $40/month per child. There is a one-time $60 registration fee, which is waived for Founding Members. Visit our Services page for complete pricing details.

What is NOT included in membership?

Membership does not cover hospitalizations, emergency room visits, specialist referral costs, advanced imaging (MRI, CT scans), or surgeries. However, Courtney coordinates all referrals and helps you navigate the process — including finding affordable options. Labs are available at wholesale cost (cost + 15%), which is a significant savings compared to traditional billing.

Do I need insurance if I join?

Insurance is optional but recommended for major medical events. DPC pairs well with a catastrophic health plan, high-deductible plan, or health sharing ministry. Your DPC membership handles day-to-day primary care — insurance is there for the big stuff. Courtney will help you think through what combination makes sense for your situation.

Can you be my only provider?

DPC covers approximately 90% of primary care needs. For hospitalizations, major emergencies, or specialist procedures, you’ll need additional coverage. But for everything else — sick visits, wellness exams, chronic conditions, women’s health, in-office procedures, lab work — Ash & Elm has you covered.

How do I contact you between visits?

You can reach Courtney directly via text, phone call, or secure message. No call centers, no automated menus, no waiting on hold. Courtney responds personally — because that’s what a real provider relationship looks like.

What if I need a specialist?

Courtney coordinates all specialist referrals and helps you navigate the process. She’ll help you find providers, understand your options, and ensure continuity of care. Referral coordination is included in your membership.

Are labs included?

Labs are available at wholesale pricing. This is typically a fraction of what you’d pay through traditional insurance billing or hospital-based labs. Many common panels cost $5–$30 at wholesale, compared to hundreds of dollars billed through insurance.

Do you accept Medicare or Medicaid?

No. Ash & Elm Wellness does not participate in Medicare or Medicaid programs. As a Direct Primary Care practice, we operate outside the insurance system entirely. Members pay their monthly fee directly — no insurance billing is involved.
